Required Immunizations and Exemptions
Texas state law requires that students under the age of 22 submit documentation of current MCV4 Bacterial Meningitis Vaccination. Documentation is due by May 15. Failure to submit documentation will result in a hold being placed on your account. You will not be able to move into the residence hall or attend classes if the hold is in place.
Routine Immunizations REQUIRED for enrollment are:
- Tetanus-Diphtheria (Td) or Tetanus-Diphtheria-Pertussis (TDaP) - within 10 years
- Measles, Mumps, Rubella (MMR)- two doses (both after first birthday)
- Meningococcal Vaccine (Menactra or MenQuadFi) - for all entering students under age 22 (within 5 years and 10 days from first day of class)

Requesting An Exemption
New students must use the official Texas Department of State Health Services (DSHS) form. The DSHS form may be ordered electronically. It is then mailed from DSHS to the student. The DSHS form must be completed, notarized, and mailed to Baylor Health Services. It is the student's responsibility to complete the DSHS form and have it notarized. A written request must be submitted for the affidavit via one of the following:
