Medical Clearance Instructions
Baylor has transitioned to a new Health Portal-based method for documenting Medical Clearance data; please follow the instructions in your goBaylor checklist and call 254-710-1010 if you have any questions.
If you an incoming graduate, law, or seminary student, and are unable to complete your Medical Clearance in the Health Portal, you may fill out and submit the old Health Form:
- Fill out your demographic data in the first section. Make a copy of the front and back of your health insurance card and send with the health form.
- Fill out your medical history, including explanations of positive answers and all medications that you take regularly.
- Sign and date the form. If you are under 18, have your parent or guardian sign and date it also.
- Fill in dates for the required immunizations for Baylor. Also attach a copy of your official immunization record (school record or doctor’s office record with healthcare personnel signature). If you do not have an official record, healthcare personnel must sign your Health Form to authenticate the immunization dates.
- Answer the 4 questions regarding possible tuberculosis exposure. If any of the answers is “yes”, you will need to go to your doctor or a public health department to have tuberculosis testing done. Either a skin test or blood test is acceptable. If the skin test is positive, you may choose to just have the chest x-ray and discuss with your doctor about being treated with INH or rifampin. You may also choose to have a blood test to make sure the skin test is accurate. If the blood test is negative, you do not need a chest x-ray. This section needs to be filled out completely and signed by a healthcare provider.

Medical Clearance FAQs
Medical records staff still need to verify the dates on the vaccine records you've submitted, and depending on the time of year, that can take up to a week. If it has been over a week and you've double-checked to make sure you've completed all the tuberculosis risk screening questions, please feel free to reach out to us at 254-710-1010.
Your housing application and roommate assignment will not be affected by pending vaccine data. However, you WILL need to submit proof of a current bacterial meningitis vaccine or an exemption affidavit before you're able to move into your residence hall!
Error messages sometimes occur if you've been seen at the Health Center before being admitted as a student (e.g. as a Summer camper or dependent of a Baylor employee). Whether or not this is the case for you, please call the Health Center at 254-710-1010 and one of our managers will investigate!
All incoming students are required to complete the Tuberculosis Risk Screening questionnaire as part of their Medical Clearance process. Depending on your answers to those questions, you may or may not need to provide proof of a recent TB test.
There are a number of vaccine options available for meningitis, and if your're unsure, the best thing will be to ask your healthcare provider. Texas state law requires a bacterial meningitis vaccine covering serogroups A,C,W, and Y. Meningitis B vaccines like Trumenba or Bexsero, while advisable, do not fulfil the state requirement. Some newer vaccines like Penbraya and Penmenvy cover all five serogroups A, B, C, W, and Y and will fulfill vaccine requirements. (Keep in mind that since Penmenvy and Penbraya require two doses each, six months apart, depending on when you get your first dose, you may not be fully immunized by the time you come to campus.)
Different doctor's offices, medical systems, and countries may all record vaccines a little differently. Meningitis vaccines may show up on your immunization records generically as "meningococcal," "MCV4," or "MenACWY," or under a trade name (MenQuadfi or Menveo, for example.)
